Upcoming Healthy Living Workshops

Washington County Public Health is pleased to offer two upcoming Healthy Living Workshops for Washington County Residents:

Healthy Living: Pathways to Positive Change

Six weekly sessions designed for anyone with a chronic health problem, as well as family members, friends, and caretakers

Build confidence and take control of your health!

When: Every Wednesday from September 16 to October 21, 9:30AM – 12NOON

Where: Cossayuna Fire Department - 21 Bunker Hill Rd, Greenwich

In partnership with Get Healthy North Country and Comfort Food Community, Washington County Public Health is pleased to offer a FREE 6-week Healthy Living: Pathways to Positive Change Workshop for Washington County residents. The workshop is an SMRC Evidence-Based Self-Management Program originally developed at Stanford University and is recommended for adults with at least one chronic health condition and/or their family members, friends, or caretakers. Chronic diseases may include but are not limited to arthritis, lung problems/diseases, cancer, stroke, hypertension, anxiety/depression, obesity, heart disease, and tobacco-use-related conditions.

The workshop, formerly known as the Chronic Disease Self-Management Program, is free to attend and each participant will receive supportive program materials including Self-Management Toolkit Resources, Action Planning Resources, a Living a Healthy Life with Chronic Conditions book and more! Washington County Public Health is also pleased to partner with Comfort Food Community, who will provide participants with weekly incentives throughout the workshop series.

The peer leaders will utilize various interactive activities to help participants acquire new skills in self-managing their chronic diseases. Session topics include:

  • Coping with fatigue, pain, and stress
  • Exercise for flexibility and endurance & Nutrition and healthy habits
  • Communicating with family and healthcare providers
  • Action-planning, problem solving, and decision making to manage daily challenges
  • Setting realistic goals and tracking progress
  • Building confidence to make informed health choices

This interactive program aims to increase confidence, physical and psychological well-being, and knowledge of chronic disease self-management strategies. This program does not, however, replace any existing programs or treatments. Feedback from previous participants include:

Living Well with Diabetes: Pathways to Control Your Blood Sugar

Six weekly sessions designed for those living with diabetes and people who are at risk of developing diabetes, as well as family members, friends, and caretakers

Gain confidence in managing diabetes and preventing complications!

When: Every Tuesday from November 10 to December 15, 1:30-4PM

Where: Washington County Burgoyne Avenue Campus - 1153 Burgoyne Ave, Fort Edward

In partnership with Get Healthy North Country, Washington County Public Health is pleased to offer a FREE 6-week Living Well with Diabetes: Pathway to Control Your Blood Sugar Workshop for Washington County residents. The workshop is an SMRC Evidence-Based Self-Management Program originally developed at Stanford University and is recommended for adults living with diabetes, and people who are at risk for developing diabetes, as well as family members, friends, and caretakers.

The workshop, formerly known as the Diabetes Self-Management Program, is free to attend and each participant will receive supportive program materials including Self-Management Toolkit Resources, Action Planning Resources, a Living a Healthy Life with Chronic Conditions book and more!

The peer leaders will utilize various interactive activities to help participants acquire new skills in self-managing their chronic diseases. Session topics include:

  • Blood sugar control techniques
  • Healthy eating and medication use
  • Exercise for strength and endurance
  • Action-planning, problem solving, and decision making for daily diabetes management
  • Monitoring and adjusting lifestyle behaviors
  • Preventing complications through proactive self-care

This interactive program aims to increase confidence, physical and psychological well-being, and knowledge of chronic disease self-management strategies. This program does not, however, replace any existing programs or treatments. Feedback from previous participants includes:

Participants are encouraged to bring their own water/drinks and a snack. Stretch breaks will be offered throughout the workshop. The workshop is currently offered to Washington County residents only.
